 This week’s first assignment is STEP 3 – Literature Review and Critical Appraisal.The student will complete the following items and submit a Word doc to the assignment link. 

1. Provide a title that conveys or describes the assignment.
2. Literature Review – Provide the key terms used to guide the search for the evidence and provide at least five (5) summaries of research studies to support the evidence.
3. Critical Appraisal of Literature – Discuss the strengths and weaknesses of the evidence, what is known from the evidence and any gaps in knowledge from the research evidence.

    • Author and Date of publication – has to be within the past 5 years to be used as evidence
    • Type of article published: systematic review, meta-analysis, random clinical trials, cohort studies, etc
    • EBP pyramid level of evidence (Ranks are levels I – VII)
    • Include what was the purpose, process, and outcomes of that research
    • Was the study appropriate and valuable to improve health
    • Who were the participants, how many, was a power analysis done to determine if statistical analysis should be performed
    • Was randomization or volunteer, or specific sample selection used
    • If a systematic review or meta-analysis, how did the authors determine what articles to choose to review?
    • What was the intervention?
    • Were the instruments used to analyze the data valid and reliable
    • What were the results? This includes the stats findings
    • What were the strengths and weaknesses of the published article?
    • This includes the EBP pyramid, the instruments reliability/validity, sample size, type of analysis done, results, limitations, etc
    • Did the strengths or weaknesses predominate and should the article be used to support your project and practice change?
    • Just because it is a level 1 on the EBP pyramid does not mean that it is a quality paper

     Title the next section: Summary of What is Known

• In this section discuss what is known from the evidence you presented and identify any gaps in knowledge from the research evidence you discussed.
• Give a closing statement

References – Cite a minimum of five scholarly references in APA 6th ed format. (7th edition is OK if that is what you have been using). This MUST be in proper APA format. 

You will perform research and appraisal of existing evidence related to challenges to the safe and effective care of the aging population in the context of global health. Select an aging population from another country, provide life expectancy, new disease patterns, longer lives, disabilities, cost of aging, health care, work, and the changing role of the family.

Read Chapter 6 & 7. Answer each question one by one.

1. Discuss the importance of effective communication in the personal relationship, the therapeutic relationship, and the relationship within the interprofessional health-care team.

7-Dr. Roberts comes into the nurses’ station demanding, “Where are Mr. Adams’s lab reports? I ordered these stat, and they’re not here! Who’s responsible for this patient?” How would you, as the nurse, respond? 

8-Explain the concept of accountability in the delegation. What are the legal ramifications of accountability in the delegation?

9. Dennie and Elias arrive in the unit for the 7:00 p.m. to 7:00 a.m. shift. Both nurses completed orientation 4 weeks ago. They find that they will be the only two RNs on the floor that night. There is a census of 48 clients. The remaining staff consists of two NAPs/UAPs and one LPN. What are the responsibilities of the RN, NAP/UAP, and LPN? Can Dennie and Elias effectively delegate client care tasks and care safely for all 48 clients? Use the Delegation Tree to make your decisions.

Competing needs arise within any organization as employees seek to meet their targets and leaders seek to meet company goals. As a leader, successful management of these goals requires establishing priorities and allocating resources accordingly.

Within a healthcare setting, the needs of the workforce, resources, and patients are often in conflict. Mandatory overtime, implementation of staffing ratios, use of unlicensed assisting personnel, and employer reductions of education benefits are examples of practices that might lead to conflicting needs in practice.

Leaders can contribute to both the problem and the solution through policies, action, and inaction. In this Assignment, you will further develop the white paper you began work on in Module 1 by addressing competing needs within your organization.

To Prepare:

• Review the national healthcare issue/stressor you examined in your Assignment for Module 1, and review the analysis of the healthcare issue/stressor you selected.
• Identify and review two evidence-based scholarly resources that focus on proposed policies/practices to apply to your selected healthcare issue/stressor.

Developing Organizational Policies and Practices

Add a section to the paper you submitted in Module 1. The new section should address the following:

• Identify and describe at least two competing needs impacting your selected healthcare issue/stressor.
• Describe a relevant policy or practice in your organization that may influence your selected healthcare issue/stressor.
• Critique the policy for ethical considerations, and explain the policy’s strengths and challenges in promoting ethics.
• Recommend one or more policy or practice changes designed to balance the competing needs of resources, workers, and patients, while addressing any ethical shortcomings of the existing policies. Be specific and provide examples.
• Cite evidence that informs the healthcare issue/stressor and/or the policies, and provide two scholarly resources in support of your policy or practice recommendations.

 How does the practice (DNP) scholar determine the fit of the framework or model to guide change? 

Answer this question with 2 paragraphs and 2 sources no later than 5 years. Remember this is related with DNP.

Students are required to submit weekly reflective narratives throughout the course that will culminate in a final, course-long reflective journal due in Topic 10. The narratives help students integrate leadership and inquiry into current practice.

This reflection journal also allows students to outline what they have discovered about their professional practice, personal strengths and weaknesses, and additional resources that could be introduced in a given situation to influence optimal outcomes. In this assignment, you will write a paper that analyzes a scenario and explains the purpose of hypotheses and research questions.

Step 1: Read the following scenario.

You are a nurse on 5 West who has concerns about visiting hours in the Critical Care Unit. The unit currently has a visiting-hours policy that allows patients to rest and the staff to provide care in an environment that protects patient privacy and modesty. However, many nurses now feel that family presence improves patient outcomes. You and other nurses on your unit have noted that vital signs are improved and patients are more responsive when family is present. You are considering whether to use a quantitative or qualitative study to research this topic.

Step 2: Consider the following questions:

• What is the purpose of hypotheses and research questions?
• Why is it important to develop workable hypotheses and research questions in conducting research?
• What type of research—qualitative or quantitative—would be most conducive to the scenario in Step 1? Why?
• Which would be more appropriate given the scenario in Step 1: the development of a hypothesis or a research question or both? Why?

Step 3: Write a two-four page paper.

Write a paper that describes the purpose of hypotheses and research questions. Articulate the importance of developing workable hypotheses and research questions in conducting research. Based on the provided scenario, describe whether a qualitative or quantitative research study would be most appropriate and why. Explain whether, and why, you would develop a hypothesis or a central research question for this scenario.

Construct your paper and cite any sources in APA format.

Read the Stokowski article from Week 3.  Consider the implementation of the EHR in your facility.  After reading the article choose one issue or argument presented.  Answer the following points:

• Discuss how you encountered this in your facility.
• What were the common responses of staff and providers? What were the barriers and how could you overcome them?
• Consider how you would have changed the implementation and/ or ongoing support for the EHR. What specific measures or support would you have provided and why? 
• How can you evaluate the effectiveness of the measures?
• If you could tell your head of information technology how to improve it or change, what would it be and why?
