CLASS DISCUSSION REPONSE

Peer initial posting:

Many Who Buy ACA Health Plans For 2019 Find Lower Prices And More Choice focuses on the stabilization of the health care market after the removal of some “structural underpinnings” in 2017. The cost for health insurance rose steeply from 2017 to 2018 with the average premium going up by over 10% for most states (Rau, 2018). A portion of the price increase was driven by insurance monopolies, so, with no competition, insurance companies had no fear about raising premiums while reducing coverage.

The cost of health care has been a hot topic for a long time, especially as prices have been going up. The cost to insure a single person in 2018 has become similar to the average cost of family coverage 20 years ago (NCSL, 2018). For many people, the question of health care is how much is a person willing to pay for their health on average, which becomes problematic very quickly as the averages can be relatively easily skewed. Even for people who receive health insurance through work, the average cost of insurance premiums is $5,714 and additional out-of-pocket costs, which bring up the average spending on health to 11.7% of total income per year (NCSL, 2018). While there is good news for the public at large at the reduced costs for 2019 insurance plans, there are still many problems with health insurance. Insurances only cover certain providers, hospitals, or services, and for people with established providers, this may mean finding a new provider or paying more for their insurance, and those are only options if there is even an insurance company that works with that provider. Overall, I am not optimistic about a system where the cost of a service can be difficult to obtain and the response of most customers is basically “at this point, I’ll take whatever I can get” (Rau, 2018).

A systematic review is a referred to as a detailed, systematic and transparent means of gathering, appraising and synthesizing evidence to answer a well-defined question. The hallmark of systematic reviews is that they seek to reduce bias at all stages of the review process and strive to gain reliable answers to probable research-oriented questions through the means of summarizing and collecting all the available valid proofs deemed to be empirical as they align with the previously stated eligible criterion (Gopalakrishnan, 2013). This type of review is often written by a panel of experts after reviewing all the information from both published and unpublished studies, which distinguishes it from traditional literature reviews that typically examine a much smaller set of research evidence and present it from a single author’s perspective.

A meta-analysis is a statistical procedure for combining numerical data from multiple separate studies. A meta-analysis should only ever be conducted in the context of a systematic review. The aim of a meta-analysis is to derive a conclusion with increased power and accuracy than what could not be able to achieve in individual studies. A meta-analysis is a systematic study in which therapeutic effectiveness is measured by calculating the weighted pooled estimate for treatments in at least two different research.

Basically a systematic review is a method used to describe trends in the research field by calculating how many studies have used certain research methodologies whereas a meta-analysis combines results from those studies in a new statistical framework to test hypotheses. Both methods can be used together, which is advantageous. However, well planned the systematic review or meta-analysis is, if the quality of evidence in the studies is low, the quality of the meta-analysis decreases and incorrect results can be obtained.

Quantitative and Qualitative methods are both used to evaluate evidence. Qualitative research focuses in understanding a research query as a humanistic or idealistic approach (Pathak et al., 2013). Though quantitative approach is a more reliable method as it is based upon numeric and methods that can be made objectively and propagated by other researchers (Pathak et al., 2013). Qualitative method is used to understand people’s beliefs, experiences, attitudes, behavior, and interactions (Pathak et al., 2013). It generates non-numerical data (Pathak et al., 2013). When combined with quantitative measures, qualitative study can give a better understanding of health-related issues (Pathak et al., 2013). The perspectives in clinical research should highlight advances in qualitative research as well, to optimize quality and utility of this method of research (Pathak et al., 2013). Qualitative research gives voice to the participants in the study, it permits the participants to share their experiences of the effects of the drug of interest. This can open our eyes to new aspects of the study and help modify the design of the clinical trial (Pathak et al., 2013). Qualitative study enhances the involvement of everyone related to the study, the researcher works on the social parameters in addition to the quantitative measures in the study. The subjects also have an empowering experience in the study (Pathak et al., 2013). They have an active role in the study and can voice their individual benefits and harms of the study, as well as, with qualitative methods, the relationship between the researcher and the participant is often less formal than in quantitative research (Pathak et al., 2013).

Alternative discussion

How am I doing? I am grateful to be able to seat at my desk and type a response to this discussion question. A mere two weeks ago, this was not possible! COVID-19 has had quite an impact on me, my family, and my work. Since the last week of October, one member of my household has tested positive for the coronavirus, been hospitalized, and on a ventilator for 5 days. The rest of us in the house have remained negative but we were taken physically to varying degrees. I am currently on a medical leave of absence from work and dealing with home health care, physical therapy, occupational therapy, and trying not to get tangled up in my oxygen tubing. I am making slow but steady progress. As I do this, I am caring for and cautiously watching over my daughter who is 38 weeks pregnant and recovering from pneumonia. My son-in-law pretty much has his hands full with their two-year son. Every day I am fielding calls and emails from my students as well as the instructors who are taking over my teaching responsibilities.

Are you able to practice self-care in this time of crisis? I learned a long time ago as a single parent that recharging my own battery was a necessity that I could not neglect. So every day, I take 15-30 minutes to recharge. Sometimes I do the same thing for weeks at a time. Other times something could be a one and done thing. But it is not missed because it is a priority to me. Doing a word puzzle. Coloring a picture. Journaling in a notebook. Over the past months, as things seemed to close in, the allotted time has been a struggle to find but I have consistently been amazed at the change of perspective and improvement in my mindset so this remains an important thing for me.

What ways, both professionally and personally, has your normal life changed? How are you adjusting to the new normal? I went from working 40-45 hours a week on campus to working 60+ hours a week from home back in March when the nursing program transitioned to online teaching and virtual clinicals. There was new technology to learn. There were internet issues. And for everyone, there was a total blur of the work/home balance. When this all started, I think most of us thought we would be done with this in a few months. So the longer it goes on, the more it is wearing on us and this becomes the concern. I have found that remaining in contact with my family and friends is vital. We talk about anything, everything and nothing! As the holidays draw closer there is a tendency to lament the gatherings that won’t be happening or how things will be different for various reasons. I have chosen to focus on the positive aspects of the new normal life I am in right now. Am I feeling 100% myself right now? Absolutely not but I am stronger today than I was yesterday and for that, I am grateful and blessed!

Health Care, State Strategies

State Strategies (20% of grade)

The purpose of this assignment is to familiarize students with health reform strategies adopted by states. Students will select a state health policy reform innovation and describe the rationale, how it was adopted (e.g., federal waivers, passage by state legislature), the funding structure, and (to the extent statistical data are available) its impact. Students should summarize their findings in a 1-2 page, single-spaced memo.

Sample memo attached. A memo is required, see attached sample.

A few examples of state innovations include Vermont’s single payer system, Massachusetts’ health reforms and Kentucky’s Medicaid healthcare program (none of these can be used).

Please see attached rubric.
