6021: Clinical Outcomes

Overview

Develop a data table that illustrates one or more underperforming clinical outcomes in a care environment of your choice. Write an assessment (3–5 pages) in which you set one or more quantitative goals for the outcomes and propose a change plan that is designed to help you achieve the goals.

Note: Each assessment in this course builds on the work you completed in the previous assessment. Therefore, it is recommended that you complete the assessments in this course in the order in which they are presented.

Knowing what is the best practice for our patients is very important in providing safe and effective care. Understanding best practices can help nurses identify areas of care that need to be improved. To identify areas of need, nurses must use evidence from various sources, such as the literature, clinical practice guidelines (CPG), professional organization practice alerts or position papers, and protocols. These sources of evidence can also be used to set goals for improvement and best practices with an eye toward improving the care experience or outcomes for patients.

The challenge facing many care environments and health care practitioners is how to plan for change and implement changes. For, if we cannot effectively implement changes in practice or procedure, than our goals of improving care will likely amount to nothing. This assessment focuses on allowing you to practice locating, assessing, analyzing, and implementing change strategies in order to improve patient outcomes related to one or more clinical goals.

This assessment will take the form of a data table to identify areas for improvement and to set one or more outcome goals, as well as a narrative describing a change plan that would help you to achieve the goals you have set.

Assessment Instructions

Scenario

Consider the current environment. This could be your current care setting, the care settings presented in the Vila Health: Using Evidence to Drive Improvement or Vila Health: Concept Maps as Diagnostic Tools media, or a care setting in which you are interested in working.

For the setting that you choose you will need to have a data set that depicts sub-optimal outcomes related to a clinical issue. This data could be from existing sources in the course (Vila Health: Using Evidence to Drive Improvement), a relevant data set that already exists (a data set from the case study you used as a basis for your Concept Map assessment, or from your current place of practice), or an appropriate data set that you have created yourself. (Note: if you choose to create your own data set, check with your instructor first for approval and guidance.)

After you have selected an appropriate data set, use your understanding of the data to create at least one realistic goal (though you may create more) that will be driven by a change strategy appropriate for the environment and goal.

Potential topics for this assessment could be:

  • Consider ways to help minimize the rate of secondary infections related to the condition, disease, or disorder that you focused on for your Concept Map assessment. As a starting point you could ask yourself, “What could be changed to facilitate safety and minimize risks of infection?”
  • Consider how to help a patient experiencing traumatic stress or anxiety over hospitalization. As a starting point you could ask yourself, “How could the care environment be changed to enhance coping?”

Once you determine the change you would like to make, consider the following:

  • What data will you use to justify the change?
  • How can the team achieve this change with a reasonable cost?
  • What are the effects on the workplace?
  • What other implementation considerations do you need to consider to ensure that the change strategy is successful?
  • How does your change strategy address all aspects of the Quadruple Aim, especially the well-being of health care professionals?
  • Once the change strategy is implemented, how would you evaluate the efficiency and effectiveness of the care system if the desired outcomes are met?

Instructions

Your assessment submission should include a data table that illustrates the current and desired states of the clinical issue you are attempting to improve through your application of change strategies. Additionally, you will need to explain the rationale for your decisions around your chosen change strategies, as well as how the change strategies will be successfully implemented. The bullet points below correspond to the grading criteria in the scoring guide. Be sure that your change strategy addresses all of them. You may also want to read the Change Strategy and Implementation scoring guide and Guiding Questions: Change Strategy and Implementation to better understand how each grading criterion will be assessed.

Topic 1: Vulnerable Populations

As you discovered in this week’s lectures and textbook readings, numerous vulnerable populations exist in the community.

Some vulnerable populations may have diseases, health impairments, and pregnancy that bring humiliation and predisposed judgment. Unfortunately, these health conditions can cause vulnerable populations to become ostracized by the general public.

Group Project

 

A common task in healthcare involves a team assessing how changing regulations, mandates, or market conditions will impact an organization.  At times, the very survival of the entity depends on this analysis.  Each student will be assigned to a team and tasked with working together to develop a presentation on the following topic:

Since the 2016 presidential election, there have been multiple changes to the Patient Protection and Affordable Care Act (ACA).  What are they and how have they impacted healthcare access, cost, and quality?

Questions to be addressed in your research include:

·                  What is the status of those who were newly covered?

·                  What are exchanges and how are different States approaching them?

·                  What does the Medicaid program have to do with the ACA?

·                  How have health insurance premiums been affected?

·                  What variables are in play in the political debate over the impact of the ACA on cost, access, and quality of care?

·                  What are some of the key challenges facing state and national policymakers given the current status?

·                  On balance, has the ACA been a good or bad law?  Provide a clear, fact-based, non-emotional justification for your conclusion.

Instructions:

Team projects occur in almost all health care worksites. This project encourages student-to-student interaction, the use of technology, and learning. You will be assigned to a team by the beginning of Week 2. 

Your project will take the form of a 10 – 15-minute multimedia presentation. You will publish your presentation to an Internet site (like Google, YouTube or your own website) and post the link to the project in the Group Presentation Assignment folder. Your presentation may contain: a PowerPoint Slideshow, a website, a video… the choice is yours. Be as creative as you can. An associated transcript with all the references is required. 

Here are some suggestions for creating and posting your project:

If you are into videos and YouTube, you likely already know how to create a video and post it to YouTube. Mac computers come with iMovie and PCs come with MovieMaker; both are easy to use tools for editing video. You can use a video camera to record your presentation. You don’t need anything fancy; the webcam on your laptop or even your phone will record video appropriate for this project. You could use a screen capture software to record the screen activities of your laptop. Quicktime Pro and Camtasia are solutions you can use for screen capture. Once downloaded and running on your machine, they allow you to record screen activity and save that recording as a movie file. There are many screen capture options available and some of them are either free or come with a free 30-day trial. Here is a web article that reviews 17 different free solutions for capturing your screen. Recording a video with either a camera or screen capture, then editing it with iMovie or MovieMaker is what I would suggest. 

Another free screen capture program, not listed at the link above, is Easyvid:  http://www.ezvid.com/.  It is well worth a look.

If you are not familiar with any of this, you can use PowerPoint to make a movie. Simply produce a slideshow in PowerPoint and record narration/timing in the slideshow settings. When you save the file, save it “as a movie”. This will create your movie file.

Uploading your presentation to YouTube is also fairly simple. If you do not have a YouTube account, it is free and simple to set up. All you need is an email address. In fact, if you have a Gmail account, you already have a YouTube account. Go to Youtube.com and register for a free account. Once you sign in, there will be an “Upload” button at the center top of your screen. Click upload and locate the movie file you saved. Be sure, in your YouTube settings, to make the movie either “Public” or “Unlisted”… No one but you will be able to see a movie that is set to “Private”. Once the movie is uploaded, navigate to it and copy the link in the address bar. Post the link to your presentation in the Team Presentation folder.

If you have trouble with any of this, there are many videos on YouTube and other tutorials on the Internet that will give you great ideas. Have fun with this assignment and be creative. You are not limited to just making a movie and posting it to YouTube. You may want to try an interesting presentation tool called Prezi. This tool allows you to create very fun, interesting, online presentations. You can even invite collaborators to create your Prezis with you. Check out the site and I encourage you to look for new and different ways to present your ideas. You can give your 10 – 15-minute presentation in any creative way you can think of.

Week 6 Assignment: PowerPoint Presentation

The following are the best practices for creating your speech presentation:

  • Title Slide: Include the title, audience (who you prepared the presentation for – school or institution), the presenter who prepared it and will be the speaker, and date.
  • Attention-Getter: Give the audience a reason to pay attention. Make them want to listen to your speech.
  • Overview: List the main ideas and sections of your presentation for the following purposes:
    • To introduce the subject and why the subject is important.
    • To preview the main ideas and the order in which they will be covered.
    • To establish the tone of the presentation.
    • To include the questions you think your audience might have about your topic – questions you will answer during your presentation.
  • Body of the Presentation: Include the information you found during your research and investigation and organize it in a visually pleasing manner.
    • Break out each main idea you will use in the body of your presentation.
    • Show some type of division like levels of headers or titles
    • Separate groups of bullets for the presentation.
    • Use a header for the title of your presentation or subtitle of the section.
    • Then, proceed to break out the main ideas. State the main idea, state major subpoints in each main idea, and provide evidence for each subpoint.
    • Cite your evidence, quotes, and statistics within your presentation, and use in-text citations in APA format.
  • Summary and Conclusion: Summarizing is similar to paraphrasing but presents the gist of the material in fewer words than the original. An effective summary identifies the main ideas and major support points from the body of your outline or presentation. Minor details are left out. Summarize the benefits of the ideas and how they affect the thesis statement of the outline and main objective of the presentation.
  • References: Use the APA citation format. The illustrations should be included with your resources. APA tutorials are available in the Chamberlain University library.
    • At least 4 authoritative, outside scholarly sources are required from Week 5 outline. (Anonymous authors or web pages are not acceptable.)
    • Appropriate citations within the presentation are required on the last slide. Just copy and paste this from the last page of your outline – the references page. References should be in APA format.
      • Each resource should be entirely double spaced.
      • All entries must use hanging indents – the first line is flush left, and all the rest are indented.
    • All Chamberlain University policies are in effect including the plagiarism policy.
