Assessment/Interview

Select a community of interest in your region. Perform a physical assessment of the community.

1. Perform a direct assessment of a community of interest using the “Functional Health Patterns Community Assessment Guide.”

2. Interview a community health and public health provider regarding that person’s role and experiences within the community.

Interview Guidelines

Interviews can take place in-person, by phone, or by Skype.

Develop interview questions to gather information about the role of the provider in the community and the health issues faced by the chosen community.

Complete the “Provider Interview Acknowledgement Form” prior to conducting the interview. Submit this document separately in its respective drop box.

Compile key findings from the interview, including the interview questions used, and submit these with the presentation.

PowerPoint Presentation

Create a PowerPoint presentation of 15-20 slides (slide count does not include title and references slide) describing the chosen community interest.

Include the following in your presentation:

1. Description of community and community boundaries: the people and the geographic, geopolitical, financial, educational level; ethnic and phenomenological features of the community, as well as types of social interactions; common goals and interests; and barriers, and challenges, including any identified social determinates of health.

2. Summary of community assessment: (a) funding sources and (b) partnerships.

3. Summary of interview with community health/public health provider.

4. Identification of an issue that is lacking or an opportunity for health promotion.

5. A conclusion summarizing your key findings and a discussion of your impressions of the general health of the community.

The purpose of dramatic arts often extends far beyond the words written on a page; staging, lighting, costumes intonation, expression, and audience experience are all part of the experience of a theatrical production. To develop your understanding of the cultural role of the dramatic arts, you will view a production of a play this week and write a reflection to explain your viewing experience. Interprofessional Collaboration

 

Medication errors can lead to adverse drug events, which are associated with increased total hospitalization cost and length of stay (McCarthy, Tuiskula, Driscoll, & Davis, 2017), with an estimate of 44,000 Americans from medical errors (Hanifin, & Zielenski, 2020). Slight, Seger, Franz, Wong, & Bates (2018) estimated a national cost projection of $871 million to $1.8 billion to treat avoidable adverse drug events. Hanifin and Zielenski (2020) found that addressing the medication error problem with initiatives supported by an interdisciplinary team has resulted in a successful decrease in medication error rates.

At the clinic where I work, I would have the therapist, nurses, clinic manager, pharmacist, physician/advanced practice provider, and information technologists on my team. The nurses are responsible for making sure the medications are sent properly and the client understands  and  must be sure that the correct medication is dispense to the patient as ordered by the provider. The provider’s role is to order the correct medication as appropriate for each patient’s clinical condition. The pharmacist is available to consult if a provider is unsure of what to order, or if a nurse has questions about dose safety or how it is to be administered. The information technologist’s role is to provide medication entry/charting in the patient’s electronic medical record and relay the findings to the clinic manager. The nurse’s role is to review the IT-generated report for accuracy and report results to the practice.

Teamwork is best achieved through communication and understanding, as well as respecting each team member’s roles, responsibilities, and concerns (White, 2016). Each member of the team should come together initially to delineate his/her unique responsibilities. With the clarity of each other’s roles and responsibilities, members should hold one another accountable and collectively answer to the leader of the collaboration, which typically would be the nurse and the provider who holds responsibility for the safety initiative and tasking clinical staff to find evidence-based solutions to the problem.  My workplace has 5-minute, daily huddles that take place at 8 in the morning before our workday begins. Each huddle session summarizes issues from the previous workday and highlights essential events/patients for the same day and the day after. This huddle session would be a great way to follow-up on divided tasks outlined in the paragraph above.

White & Dang (2018) highlighted a critical barrier – time. Each member of the team will need uninterrupted time to think about and discuss the problem. Biweekly or weekly staff meetings, spanning 30-60 minutes, are ideal for each team member to have the opportunity to voice their concerns.  Also, the team leader should encourage everyone to voice their concerns publicly and privately for effective communication and collaboration. Above all, the team leader must encourage each team member to value one another’s professions, interests, concerns, and responsibilities (White & Dang, 2018).

When you wake in the morning, you may reach for your cell phone to reply to a few text or email messages that you missed overnight. On your drive to work, you may stop to refuel your car. From the moment you wake, you are in fact a data-generation machine. Each use of your phone, every transaction you make using a debit or credit card, even your entrance to your place of work, creates data. It begs the question: How much data do you generate each day? Many studies have been conducted on this, and the numbers are staggering: Estimates suggest that nearly 1 million bytes of data are generated every second for every person on earth.

As the volume of data increases, information professionals have looked for ways to use big data—large, complex sets of data that require specialized approaches to use effectively. Big data has the potential for significant rewards—and significant risks—to healthcare. In this Discussion, you will consider these risks and rewards.

“Meaningful Use” is applied to the extent to which a health care provider/organization uses electronic health records (EHR). Defined as; Use of certified EHR in a meaningful way, the use of certified EHR technology for the electronic exchange of health information to improve quality of health care, and the use of certified EHR technology to submit clinical quality reporting and other measures (Glaser, 2018).

EHR provides accurate and complete information about patients’ health. Hence, the health care provider is able to provide the best care possible. Coordination for the care provided by the health caregiver will be more efficient. An effective way of sharing information with the patient together with his/her family. Significantly the health care providers will have the necessary data to diagnose the health problems sooner, reduce medical errors, and also care will be provided at a lower cost. Not forgetting communication between health professionals and health entities will be more efficient and effective.

The data is captured in coded format and expands the exchange of information in the most structured way to elevate clinical processes and better health outcomes by placing more emphasis on clinical conditions that are of priority, patient self-management, and access to comprehensive data. It can be related when one is taking care of patients. There is always more emphasis on critical conditions, and the patient’s self-management has to be into consideration. Access to comprehensive data about the patient is also of significance when taking care of a patient.

I had a patient who was checking in for his checkups after two weeks. Keeping records of his progress was necessary at this particular moment. Thanks to EHR, coordination for the care I was to provide each time he visited was on track, which led to acceleration on his recovery. I have not encountered any negative impact that comes with the use of EHR. It’s a reliable and effective program that leads to better health care services.

Electronic health records (EHRs) are used across clinical care and healthcare administration to capture a variety of medical information from individual patients over time, as well as to manage clinical workflows. (Ehrenstein et al, 2019) According to the article, the use of electronic health records (EHRs) will improve patient care, decrease practice costs, and increase provider productivity and revenue. (Chin & Sakuda, 2012) At my work, EHRs help nurses and doctors work effectively and productivity. By using EHRs, we can share patient’s medical history, medications, labs, and progress notes, so we can just check it and provide necessary care right away to patients without asking other medical professionals about patient’s information. In addition, data shared in an EHRs can help clinicians choose the right medication for a patient with allergies (Steger, 2017), and it prevents medication errors. 

The barriers to the implementation of EHRs include potentially high financial investments, an increase in initial physician and staff training time, workflow redesign efforts, and the need to hire new staff for HIT support, an EHR creates a database of information that will assist in the coordination of patient care and improvement of communication about shared patients among health care providers. (Chin & Sakuda, 2012) At my work, I have seen some nurses and doctors were struggling with using EHRs because they used to do paper documentations about their patient care. They had to take extra time to complete documentation. In addition, sometimes the information of EHRs is not updated timely, and it makes miscommunication between doctors, nurses, and other healthcare professionals.
