 In my opinion, I will recommend that this patient would be assessed and treated for Post-Traumatic Stress Disorder (PTSD) as well. I believe the stress life of this patient regarding her job (compulsory overtime), and relationship issue (divorce) can be a precipitating factor for the development of PTSD. According to Engelhard et al (2007), substantial symptoms of PTSD can arise from relatively minor stressors such as problems with work, finances, and relations. Besides, the hallmark of PTSD, anxiety disorder, and depression are caused by the same intrusive memories. Thus, symptoms of PTSD, anxiety, and depression overlap (Engelhard et al 2007).

Besides, this patient should be taught to practice diaphragmatic breathing techniques (deep breathing). Per Ramsey (2019), breathing is the number one anxiety fighter because it soothes the nervous system. The respiratory system is controlled by the autonomic nervous system which consists of the sympathetic and parasympathetic nervous systems. During a period of anxiety, the sympathetic nervous system is activated causing rapid breathing, elevated heart rate, tremor, diaphoresis, poor concentration, and the like. All these happen because there is a limited amount of oxygenated blood in the body system. However, when the victim is encouraged to practice deep breathing exercises, the diaphragm is elevated, giving room for more oxygen to enter the bloodstream. This stimulates the parasympathetic nervous system to activate leading to reduced respiration, heart rate, fear, diaphoresis which eventually end up in anxiety reduction.

The Enterprise Risk Management Domains are a great tool to assess and coordinate surveillance plans in order to operate a hazardous free facility. I work in an Alzheimer’s and Dementia facility; in addition to the general risk plans, we have to add behavioral, fall prevention, and special disaster preparedness due to the patients’ inability to ambulate and verbalize needs. I will choose the two domains that we emphasized the most Clinical & Patient Safety and Environmental- and Infrastructure-Based Hazards. Clinical & Patient Safety – The facility prepares diligently to identified and respond to threats that may affect the overall health of the residents; they have designated team members to monitor patients outcomes in regards to antibiotic use, infection control and Healthcare Acquired Infections (HAI), that team member is called an infection preventionist, she is in charge of performing rounds to each department (laundry, nutrition, activities, nursing, etc.) to ensure they are in compliance with infection control guidelines and protocols. Infrastructure-Based Hazards – The environmental and maintenance team is of great support when it comes to environmental hazards, disaster preparedness, and patient safety; the facility is designed to protect the resident by preventing injuries, such as falls, drowning, choking, and aspirating. We are a lockdown unit to prevent elopement, the sinks are in special units to prevent patients from falling or ingesting the soap, their rooms have limited furniture, and the patios are gated with locks. I believe their plan is very effective in mitigating risk; I will suggest more assistive staff; the facility is against all types of restrains, meaning that if a patient becomes aggressive and combative, we have to handle the situation by other means, and it can be time-consuming. The quality improvement projects at the facility require a different approach due to the special infrastructure, the population, and the limited staff.

 A systemic review of literature is a structured and well-guided search for available literature that have been reviewed, free of bias, and can be recommended for practice. (Houser, 2016).  According to Houser (2016), conducting a systematic review that results in an evidence-based practice guideline requires the ability to develop research questions methodically, write inclusion criteria, conduct in-depth literature searches, and review the results of many studies critically (p.10). It is therefore important for a nurse to be well knowledgeable about ways to identify and review articles that are evidence-based and can be safely implemented during patient care. Interaction Between Nurse Informaticists and Other Specialists

 

Nature offers many examples of specialization and collaboration. Ant colonies and bee hives are but two examples of nature’s sophisticated organizations. Each thrives because their members specialize by tasks, divide labor, and collaborate to ensure food, safety, and general well-being of the colony or hive.

Of course, humans don’t fare too badly in this regard either. And healthcare is a great example. As specialists in the collection, access, and application of data, nurse informaticists collaborate with specialists on a regular basis to ensure that appropriate data is available to make decisions and take actions to ensure the general well-being of patients.

In this Discussion, you will reflect on your own observations of and/or experiences with informaticist collaboration. You will also propose strategies for how these collaborative experiences might be improved.
