My biggest weakness is my expectations of perfection in my entire environment.

As I see it, is vital for a professional nurse to have superb communication such as effective listening skills and verbal persuasion. That way, you can effortlessly comprehend the anxieties of the patients, impact their feelings, and describe treatment procedures in an understandable way to diminish resistance. Working closely with problematic coworkers can be tricky but it is essential to remember the patient, their care, and their family. Placing aside disagreements is vital for all nurses because not every person is the same. At times this implies not debating specific topics such as religion or politics at work. Not every person will become friends in the workplace but working together is important for effective care. (Gaines, 2020).

Tell me about yourself?

I am passionate about my work and profession, ambitious and driven, highly organized. I am a people person; I am a natural leader. Also, I am a results-oriented and excellent communicator and listener. I am a student who tries for the absolute best grades, obtained a summa Cum Laden in my associate and bachelor’s degrees. It is my goal for the masters and the DNP. I am greatly confident about myself. I always loved health care, But I could not study it in my country due to politics and embrace the opportunity in the USA to change my career from Cybernetics Mathematics to Nursing. Through my years of practice, I found that preventive care is my preferred clinical practice.

 Case 3

Frailty can impact an older adult’s ability to recover from an acute illness, injury and other stresses on the body.  Incorporating physical therapy and physical activity into a patient’s daily regimen, adjusting daily nutritional requirements, advising of necessary home modifications and conducting a comprehensive geriatric assessment are all interventions that can be ordered by providers in order to help prevent complications that can occur with concerns of frailty.  Older adults are more inclined to sustaining fractures due to age-related loss of muscle mass.  Physical activity and physical therapy that is age-appropriate can be very beneficial for older adults as it helps with maintaining the current muscle mass of the elderly person and they both synergistically work to reduce the risk for falls and injuries in this patient population (Cacchione, 2020).Young adults should be educated on all contraceptive options that are available so that they can decide which option will suit them best.  It is best for the individual to make their own choice on contraceptive options after being educated on all options because naturally an individual would be more likely to stay compliant with their own choice/option as opposed to one that is selected for them.  The provider’s duty to the patient is to provide accurate information to their patients and answer questions and concerns that they may have, along with remaining nonjudgmental during their interactions during the teaching sessions with the patient (Chilton, 2017).  Personally, I do not hold any convictions and judgements with regards to options that persons choose because I am a staunch believer in that we are not here to judge others.  Patients have the right to make their own personal choices as they will be living through and with the choices that they make.  As a provider, I will remain neutral with regards to patient interactions and teaching sessions on all subject matters; trust is very hard to earn and as a provider, it is imperative to gain and keep the trust of clients so that they can be cared for effectively and efficiently.A smoking cessation plan should entail the following: developing a plan with a set “quit” date; researching and selecting a nicotine replacement and other aids to help quit the habit; the individual should be able to identify and be aware of their nicotine triggers and cravings; the individual should be able to quit in the manner in which they choose to do so; and the patient should also continuously speak with their provider regarding any concerns and obstacles they may face along the way while going through the process of quitting smoking.  A great support system can aid in the person’s efforts to quit smoking (Smith, 2015).Poverty is hard on every population and can be even more so on the elderly.  Elderly adults will not be as physically healthy and/or may not have full and intact mental faculties as they once had when they were younger.   A disposition such as poverty can only make life much harder for the elderly person living in and going through this situation.  An elderly person who is on medications and needs to be monitored frequently in terms of medical management, may run into issues regarding their health if they are living in poverty.  The elderly person may also be severely depressed due to their disposition which is also of great concern as mental health conditions can be even more debilitating and wearing on the person than other physiological health conditions. Community healthcare professionals can work with political figures collectively to establish programs and resources that can help elderly constituents who are of lower socioeconomic statuses.  Community healthcare professionals know the specific needs of their patient population and they can communicate these needs directly to political figures so that resolutions to concerns can be achieved (Chilton, 2017).

Understanding the health care system at a local level means that you are aware of the culture that resides within and what causes and effects are in place for that local community. This is important when implementing evidence-based practice to ensure the health care system is functioning and providing to address the needs in a local, community, and on a neighborhood level. In the community and facility selected for the change project, as evidenced by my preceptor there are over 200 children that have asthma most using and needing controlled medication. This information is generated through a combined department of health and education system, which can filter and scan for children with health needs, based on diagnosis’ entered in the system. In speaking with several parents there are many who state their child are not asthmatic, and they “just have colds”, or that their child’s symptoms occurred only once or twice in their lifetime. Although incidents of asthma can occur once their diagnosis goes on in the system throughout their lifetime as children. School health officials get notification alerts (shown to me by the preceptor) of prior hospitalizations and diagnosis of asthma. This system allows the nurses within the school to address and follow up with children and their caretakers. Research tells us there are 770 visits to the emergency department in relation to asthma in Washington Heights (Pichardo, 2016). The department of health continues to make parents aware through campaigns such as Your Child’s Asthma Is Always There, Even When They Seem Perfectly Fine, to help them understand and remind them that asthma is a disease process that can be triggered at any given time, and not all children experience the same symptoms of severity, many children can live life without the use of controller medication whereas others need it to keep asthma in a dormant phase. Taking proper measures is important in the prevention of flare ups and hospitalizations. Families with the understanding that their children do not have asthma, because their symptoms have temporarily left them, can many times place their child at risk for emergency calls while in school, if they do not have inhalers or medication to keep them controlled. The Washington Heights/Inwood network has a mission to find and help treat children with asthma in the community, schools, clinics, and daycare organizations (Asthma Community Network, n.d.). As you can see, the community has some resources that aims to help children with asthma, and helping these children go deeper than the outside community it start in the home and within the school system to address these concerns. Together I hope to reinforce teaching and come up with solutions and/or strengthening the ones already in place by finding and filling gaps.

It is very important to understand the health care system at a local level because this will aid the nurse in determining if the new EBP will apply to the health care setting. Evidence-based practice significantly improves the quality and safety of healthcare, enhances patient outcomes, and reduces cost. EBP includes lifelong problem-solving approaches to how healthcare is delivered and integrates the best evidence from high quality studies. When planning an evidence based practice implementation, we must first understand and identify the challenges facing the healthcare system. Inadequate and delayed implementation of current evidence into practice has contributed to medical errors, safety issues, and patient deaths (Harris et al., 2015).

In talking to my preceptor about the increased rates in central venous catheter infections, she agreed that this is a recurring problem in many dialysis clinics, as she rounds in 8 different clinics. She has observed nurses performing inadequate catheter care, and has stopped to correct and guide them as to what they are missing or doing wrong. Other times she has asked patients questions about their plan for the catheter removal, catheter care, or access placement and the patients are clueless and unaware of what needs to be done, or the reason why it needs to be removed or well cared for. She agrees that this is a topic that needs to be addressed thoroughly with nurses and patients as well.
