Week 1 Midweek Assignment

In a Microsoft Word document of 2-3 pages formatted in APA style, you will describe a draftplan for your capstone project using the format below. Please note that the title and reference pages should not be included in the total page count of your paper.

Introduction

The introduction establishes the scope, context, and significance of the project being conducted by summarizing current understanding and background information (literature) about the topic, stating the purpose of the work, explaining briefly the approach used to examine the problem, and identifies one or two outcomes of the project.

Problem Statement

A problem statement is the description of a currently existing issue, which needs to be addressed with evidence-based interventions. The statement of the problem is the focal point of the project. Be sure to back up the noted issue/problem with evidence. Must include citations of evidence.

Questions

Identify one to two questions to address with your capstone project. Based on the constraint of project length of 10 weeks you may not have time to address a multitude of questions.

Methodology

The methodology section of the paper addresses:

  • when and how you will collect the pre-intervention data,
  • what the intervention(s) is (are),
  • when/how the intervention(s) will be implemented,
  • how and when you will perform your post-intervention data measurement,
  • how the data will be analyzed.

A friendly reminder that unless your project has gone through the IRB process you will not be allowed to collect data on more than 3 people. For data collection, you will choose 3 people from your aggregate.

Timetable

Develop a timetable considering the following questions:

  • When will your project start and finish?
  • Are there particular steps or stages to the project?
  • What objectives have you set for this investigation? Are they addressed in the timeline?
  • Is the timetable realistic?
  • How will you provide regular updates and progress reports and to whom will you provide them? How will you demonstrate progress?

 Cultural diversity refers to the differences among people based upon shared ideology and shared valued sets of beliefs, norms, customs, and meanings evidenced in a way of life. This diversity is expressed in many ways. Diversity in history, beliefs, practices, and opportunities not only exist among the many cultural groups, they typically exist across a wide continuum within the group. Health behaviors are influenced by culture and cultural values as well as socioeconomic status. In this assignment, students will develop a health promotion project for a cultural group living anywhere in the United States. You are to learn as much as you can about the culture norms and patterns and how these norms and patterns influence health practices. The purpose of this assignment is to develop a deeper understanding and sensitivity regarding the role of culture in patients’ lives and in nursing through the examination of a cultural/ethnic group while addressing health promotion strategies and disease prevention. PROJECT COMPONENTS The project should address the following Aspects of Cultural Assessment: 1. Ethnic/racial identity: How does the group identify itself in terms ethnicity and racial background? What is the range of interaction outside of the cultural group? Are recreational, educational, and other social activities within the ethnic reference group, the wider community, or both? 2. During my practicum, the safe transition of medical was a problem faced by the organization. Transition of care refers to the movement and coordination of care from one setting to another (AHRQ, 2018). How can we provide a safe transition of care for all patients? The whole team (doctors, nurses, case managers, PT/OT, pharmacist, dieticians, and specialist) plays a critical role in planning for a safe discharge and ensuring a smooth transition of care from hospital to home or other care settings, which should start on the day of admission. Care providers must communicate important information to the patient, families, caregivers, and among themselves in a timely manner. Physicians must ensure that patient understand their medical conditions/plan of care, coordinate patient’s health care to various settings and providers and receive enough knowledge and resources upon discharge to home or other healthcare settings (The Joint Commission, 2012). Case managers collaborate with the interdisciplinary team to discuss patients’ needs such as SNF placement, home health care, DME, transfer to high level of care, home PT/OT, order medical supplies, IV antibiotics, and ensure patient has a safe place to recover. Nurses must ensure that patient/families/caregivers receive a clear discharge instruction including recommendations, medication regimens, follow-up care, education on self-care, warning signs of worsening conditions, who to contact in case of emergency, and how to promote health and prevent illness in the patient’s preferred language (The Joint Commission, 2012). Providing a safe and effective transition of care from the hospital to home or other health care settings prevent readmission and adverse events, which is the care team’s responsibilities.
