 Purpose

The purpose of this assignment is to provide the student an opportunity to reflect on the weekly concepts learned in the course.

Activity Learning Outcomes

Through this assignment, the student will demonstrate the ability to:

  1. Examine roles and competencies of advanced practice nurses essential to performing as leaders and advocates of holistic, safe, and quality care (CO1)
  2. Apply concepts of person-centered care to nursing practice situations. (CO2)
  3. Analyze essential skills needed to lead within the context of complex systems. (CO3)
  4. Explore the process of scholarship engagement to improve health and healthcare outcomes in various settings (CO4)

Requirements:

Reflection: write 1-2 paragraphs reflecting on your learning for the week. Guiding questions are provided or you may write about what you felt was most significant to you for the week.

You will need to post your reflection here before you are able to see other students’ posts.

  • Nearly every student in the course was able to think of a time when they had personally been impacted by or had observed incivility in the workplace. In what ways does this impact your view of the nursing profession? If those outside of healthcare were aware, how might it impact their view?
  • As you move towards your role as an NP, what will you do to support healthful work environments for yourself and your colleagues?
  • Think about your personal use of social media. Are there changes you would like to make in how you use social media?
  • What self-care practices do you engage in to promote personal holistic health?

Educational Program on Risk Management – Part One: Outline of Topic

The purpose of this assignment is to create an educational program that supports the implementation of risk management strategies in a health care organization.

In this assignment, you will develop an outline for an “in‐service”‐style educational risk management program for employees of a particular health care organization that will then form the basis for a PowerPoint presentation in Topic 5. Select your topic for this educational session from one of the proposed recommendations or changes you suggested in the Risk Management Program Analysis – Part One assignment to enhance, improve, or secure compliance standards in your chosen risk management plan example.

Create a 500‐750-word comprehensive outline that communicates the following about your chosen topic:

  1. Introduction: Identify the risk management topic you have chosen to address and why it is important within your health care sector.
  2. Rationale: Illustrate how this risk management strategy is lacking within your selected organization’s current risk management plan and explain how its implementation will better meet local, state, and federal compliance standards.
  3. Support: Provide data that indicate the need for this proposed risk management initiative and demonstrate how it falls under the organization’s legal responsibility to provide a safe health care facility and work environment.
  4. Implementation: Describe the steps to implement the proposed strategy in your selected health care organization.
  5. Challenges: Predict obstacles the health care organization may face in executing this risk management strategy and propose solutions to navigate or preempt these potentially difficult outcomes.
  6. Evaluation: Outline your plan to evaluate the success of the proposed risk management program and how well it meets the organization’s short-term, long-term, and end goals.
  7. Opportunities: Recommend additional risk management improvements in adjacent areas of influence that the organization could or should address moving forward.

My practicum takes place at a child daycare center. This past week I spent time with the four and five year- old children. We talked about our teeth. When asked to raise their hand if they brushed their teeth, I was surprised to find that out of fifteen children, only eight had. This provided a good teaching opportunity about the function of our teeth.

We looked at pictures of teeth and learned that they help us bite, chew, talk, and help us smile. Then I read the book Why Should I Brush My Teeth. They were so enthralled with the twelve-page story. Afterward, I answered some of their questions about their teeth.

One of the Healthy People 2020 goals is to prevent dental caries. I believe that childcare centers can serve as educators for oral health. This can be achieved by offering water instead of fruit juice and sending home flyers to parents about healthy foods.

I learned that I must shorten my lessons. Children ask a lot of questions, and I felt as if I needed to answer them all. My preceptor kept pointing to her watch. I do not know why I thought this would be easy.

This week was rather interesting. Just a reminder I am doing my capstone at A.L.I.C.E hope a non-profit organization that helps people deal with different trauma they are going through with the main one being domestic violence. My preceptor asked me to attend a banquet where she was getting honored for all her work within the community. This week my preceptor went met with different community leaders to discuss a plan to help victims of domestic violence. She also told me that she is a two-time survivor of domestic violence and that this is something close to her heart and she wanted women to know that they can get away and feel safe.

We had a client call and said she was feeling really down and felt like everywhere she went her partner was following her. She said she started receiving calls and when she answered the person hung up in her face. She said she felt like she can’t live like this anymore. We met up with her and helped get her numbers changed. We also set her up with temporary housing with a security system so she feels a little safer. My preceptor also talked with her to see where her mind was because she knew other women that committed suicide and did not want this to happen. The lady says he feels safer and is going to set up an appointment with the therapist. At that moment I never realized how traumatic or emotionally draining. This made me want to help even more. Everyone deserves to feel safe at home and not constantly looking over their shoulder. Women who have dealt with domestic violence may have depression or even experience PTSD. October is domestic violence awareness month and my preceptor mentioned to me that many people think domestic violence is only physical and she says that a myth. She said domestic violence is physical, mental, verbal, emotional, financial, and psychological abuse.

Chronic Stable Angina

E.H. is a 45-year-old African American man who recently moved to the community from another state. He requests renewal of a prescription for a calcium channel blocker, prescribed by a physician in the former state. He is unemployed and lives with a woman, their son, and the woman’s 2 children. His past medical history is remarkable for asthma and six “heart attacks” that he claims occurred because of a 25-year history of drug use (primarily cocaine). He states that he used drugs as recently as 2 weeks ago. He does not have any prior medical records with him. He claims that he has been having occasional periods of chest pain. He is unable to report the duration or pattern of the pain. Before proceeding, explore the following questions: What further information would you need to diagnose angina (substantiate your answer)? What is the connection between cocaine use and angina? Identify at least three tests that you would order to diagnose angina.

Diagnosis: Angina

1. List specific goals of treatment for E.H.

2. What dietary and lifestyle changes should be recommended for this patient?

3. What drug therapy would you prescribe for E.H. and why?

4. How would you monitor for success in E.H.?

5. Describe one or two drug–drug or drug–food interactions for the selected agent.

6. List one or two adverse reactions for the selected agent that would cause you to change therapy.

7. What would be the choice for the second-line therapy?

8. Discuss specific patient education based on the prescribed first-line therapy.

9. What over-the-counter and/or alternative medications would be appropriate for E.H.?

Treating a Patient with Psychosis – 2 pages

This week you will build on your assessment and treatment skills as you examine clients presenting for antipsychotic therapy. In practice, clients may present with delusions, hallucinations, disorganized thinking, disorganized or abnormal motor behavior, as well as other negative symptoms that can be disabling for these individuals. Not only are these symptoms one of the most challenging symptom clusters you will encounter, many are associated with other disorders such as depression, bipolar disorder, and disorders on the schizophrenia spectrum. As a psychiatric mental health nurse practitioner, you must understand the underlying neurobiology of these symptoms to select appropriate therapies and improve outcomes for clients.

Please discuss each medication option listed in Decision Point 1. Why did you choose/not choose each medication? What is the mechanism of action for each medication? Discuss first line FDA approved medications for the disease state, taking into consideration your client’s age, sex, and ethnicity.

Consider the following scenario:

During the third week of your internship, the CIO emails your team several examples of other IT project implementations. Later that day, she says, “Review the examples I sent you about the implementation process. I attached a list of questions for you to answer about how the implementation process works and why IT projects sometimes fail. I’m most interested in your analysis of how organizations like ours can minimize the occurrences and effects of IT failures.”

Read the Case Study—Memorial Health System CPOE Implementation. (attached)

Write a 350- to 700-word paper in which you answer the following questions:

  • What is the typical IT implementation process? What are the roles and responsibilities involved in system implementation?
  • How did the process described in the case study fail to include the fundamental activities of a typical IT implementation process?
  • What are at least five indicators of project failure that manifest themselves in the case study?
  • For each indicator you described, what might you have done differently to eliminate or minimize its effect?

Format your paper according to APA guidelines.

Treatment of Psychiatric Emergencies in Children versus Adults

Treatment of Psychiatric Emergencies in Children Versus Adults

The diagnosis of psychiatric emergencies can include a wide range of problems—from serious drug reactions to abuse and suicidal ideation/behaviors. Regardless of care setting, the PMHNP must know how to address emergencies, coordinate care with other members of the health care team and law enforcement officials (when indicated), and effectively communicate with family members who are often overwhelmed in emergency situations.

In this Discussion, you compare treatment of adult psychiatric emergency clients to child or adolescent psychiatric emergency clients.

Learning Objectives

Students will:

· Compare treatment of adult psychiatric emergency clients to child or adolescent psychiatric emergency clients

· Analyze legal and ethical issues concerning treatment of child or adolescent psychiatric emergency clients

To Prepare:

· Review the Learning Resources concerning emergency psychiatric medicine.

· Consider a case where an adult client had a psychiatric emergency. If you have not had a personal experience with an adult client who had a psychiatric emergency, you can conduct an internet or library search to identify.

Post:

· Briefly describe the case you selected.

· Explain how you would treat the client differently if he or she were a child or adolescent.

· Explain any legal or ethical issues you would have to consider when working with a child or adolescent emergency case.

Assessment 1: Evidence-Based Patient-Centered Concept Map

  • Overview
Create an evidence-based, patient-centered concept map that illustrates an individualized approach to patient care, based on a patient case file of your choice.
Evidence-based practice is a key skill in the toolkit of the master’s-prepared nurse. Its goal is to ensure that health care practitioners are using the best available evidence to ensure that patients are receiving the best care possible (Godshall, M., 2015.). In essence, evidence-based practice is all about ensuring quality care.
SHOW LESSIn this assessment, you have an opportunity to apply evidence-based practice and personalized care concepts to ensure quality care and improve the health of a single patient.
By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and assessment criteria:

    • Competency 1: Apply evidence-based practice to plan patient-centered care.
      • Analyze the needs of a patient, and those of their family, with regard to how they will influence a patient-centered concept map.
      • Design an individualized, patient-centered concept map, based upon the best available evidence for treating a patient’s specific health, economic, and cultural needs.
    • Competency 3: Evaluate outcomes of evidence-based interventions.
      • Propose relevant and measurable criteria for evaluating the outcomes of a patient-centered concept map.
    • Competency 4: Evaluate the value and relative weight of available evidence upon which to make a clinical decision.
      • Justify the value and relevance of evidence used as the basis of a patient-centered concept map.
    • Competency 5: Synthesize evidence-based practice and academic research to communicate effective solutions.
      • Develop a strategy for communicating with patients and their families in an ethical, culturally sensitive, and inclusive way.
      • Integrate relevant and credible sources of evidence to support assertions, correctly formatting citations and references using APA style.

  • Questions to Consider: As you prepare to complete this assessment, you may want to think about other related issues to deepen your understanding or broaden your viewpoint. You are encouraged to consider the questions below and discuss them with a fellow learner, a work associate, an interested friend, or a member of your professional community. Note that these questions are for your own development and exploration and do not need to be completed or submitted as part of your assessment.
Recall an experience you have had—or one that you might have observed in your care setting—in which you individualized care for a patient.

    • In your approach to individualized care, did you:
      • Address any health concerns other than those for which the patient was seeking care?
      • Consider the patient’s economic and daily environmental circumstances?
      • Consider any ethical issues inherent in working with the patient?
    • What might you have done differently, if you could revisit that patient’s case?
    • What evidence supported your original course of action?
    • What evidence would you present to support an alternative course of action, if you could revisit that patient’s case?
  • Assessment Instructions: Preparation
You have been presented with a number of patient case files in the Evidence-Based Patient-Centered Care media piece. You reviewed each case, selected one case for further research, and created draft evidence-based concept map to illustrate an approach to individualized care for the patient. In this assessment, you will build upon and refine your draft concept map and develop a supporting narrative.
Create your concept map and narrative as separate documents. Be sure to note the areas where you need to include your evidence-based support and where you need to make clear your strategies for communicating information to the patient and the patient’s family.
Note: Many organizations use the spider style of concept maps (see the Taylor & Littleton-Kearney article for an example). Also, if a specific style of concept map is used in your current care setting, you may use it in this assessment.
Create your concept map and narrative as separate documents. Be sure to note the areas where you need to include your evidence-based support and where you need to make clear your strategies for communicating information to the patient and the patient’s family.
    Requirements
Note: The requirements outlined below correspond to the grading criteria in the scoring guide, so be sure to address each point. In addition, you may want to review the performance level descriptions for each criterion to see how your work will be assessed.
    Supporting Evidence and APA Style
Integrate relevant evidence from 3–5 current scholarly or professional sources to support your assertions.

    • Apply correct APA formatting to all in-text citations and references.
    • Attach a reference list to your narrative.
    • Concept Map
    • Develop a concept map for the individual patient, based upon the best available evidence for treating your patient’s health, economic, and cultural needs.
    • Narrative
Develop a narrative (2–4 pages) for your concept map.
    • Analyze the needs of your patient and their family, and determine how those needs will influence a patient-centered concept map.
      • Consider how your patient’s economic situation and relevant environmental factors may have contributed to your patient’s current condition or affect their future health.
      • Consider how your patient’s culture or family should influence your concept map.
    • Justify the value and relevance of the evidence you used as the basis of your concept map.
      • Explain why your evidence is valuable and relevant to your patient’s case.
      • Explain why each piece of evidence is appropriate for both the health issue you are trying to correct and for the unique situation of your patient and their family.
    • Propose relevant and measurable criteria for evaluating the degree to which the desired outcomes of your concept map were achieved.
      • Explain why your proposed criteria are appropriate and useful measures of success.
    • Explain how you will communicate specific aspects of the concept map to your patient and their family in an ethical, culturally sensitive, and inclusive way. Ensure that your strategies:
      • Promote honest communications.
      • Facilitate sharing only the information you are required and permitted to share.
      • Are mindful of your patient’s culture.
      • Enable you to make complex medical terms and concepts understandable to your patient and their family, regardless of language, disabilities, or level of education.
    • Additional Requirements
    • Be sure to include both documents when you submit your assessment.
    • Note: Faculty may use the Writing Feedback Tool when grading this assessment. The Writing Feedback Tool is designed to provide you with guidance and resources to develop your writing based on five core skills. You will find writing feedback in the Scoring Guide for the assessment, once your work has been evaluated.
