Diagnosis from Diagnostic and Statistical Manual of Mental Health Disorders, DSM-5 and justification

The most likely DSM diagnosis for the patient is Major Depressive Disorder, recurrent episode, severe with psychotic features.

I diagnosed the patient with Major Depressive Disorder, recurrent episode, severe, with psychotic features because of the symptoms she is exhibiting like being sad, anxious, depressed most of the time, cutting self ,suicide attempts by overdose on acetaminophen. He continues to be withdrawn to self. According to the American Psychiatric Association (2013), this diagnosis can be specific if the patient exhibits signs of anxiety, with melancholic features; or mixed with atypical features and mood-incongruent psychotic features. Also, if the patient has during the same 2-week period shown five or more of the following symptoms: In the case study the patient continues to manifest

Depressed mood example, feeling sad, empty, or hopeless (most of the day, nearly every day) 

 clearly diminished or loss of interest or pleasure in all, or almost all, activities most of the day, nearly every day

-Feelings of worthlessness as stated by patient after the breakup.

-Fatigue and loss of energy nearly every day

-Insomnia nearly every day.

-Recurring and persistent suicidal ideation without or a suicide attempt or a precise plan for committing suicide.
The above symptoms enumerated are based on what is specific on the patient from the list in the DSM-5 because this is what the patient presents with (American Psychiatric Association, 2013). Also, Oyama and Piotrowski (2013), states that depression is mostly used to express diverse feeling.

Explain group Therapeutic Approaches you might use with this Client

CBT is the treatment modality I will use to treat the client through cognitive restructuring and positive self-talk to eliminate negative thoughts. It attempts to correct cognitive distortions, particularly negative conceptions of one’s self.

Possible ways of coping with these feelings discussed along with activities that may help to reduce the symptoms.

            Ethically both the child and parents should be involved in the child’s healthcare decisions. According to Vitello (2013the parents give legal consent for treatment and thus the physician ought to carry the parent along with treatment plans, and medication management. Communication is important because any misunderstanding between parent and prescribers’ instruction can be harmful to the child. Effective communication between the parent and the provider is important because the parent is accountable to guarantee that the child is medicated as ordered and to report any adverse effect. Purnell (2014) advices that understanding communication styles, the cultural variances and values, and other communication competencies is important to be acquainted with the patients from different cultures.

Elaina, 27 years old, visits a prenatal clinic to confirm a positive pregnancy test. Elaina tells the nurse that she is not married and has no support system. She also states that she is making minimum wage at a restaurant with no health insurance and cannot afford prenatal care. She intends to keep the baby and would rather not tell her partner because “they don’t have that kind of a relationship.” Elaina asks the nurse “If I take good care of myself, why do I need prenatal care?”

#2) Explain how the fetus is affected by tobacco use during pregnancy.  Keeping in mind the 2020 National Health Goals, what are some things you can do as a nurse to promote these goals and carry your ideas out into the community.

Cultural competence is “a process for addressing disparities, based on race, ethnicity, and culture” (Isaacs et al., 2005, p. 5). Nurses must be culturally competent to provide effective and therapeutic care for patients.  It is part of a holistic treatment process. 

How does culture shape an individual’s view of mental health?

Every culture has a different way of looking at mental health and its own way of explaining mental illness which is based on a set of beliefs and practices. Due to shame and stigma associated with mental health illness and individual’s cultural beliefs, what culture has taught them on what to approach and what to avoid, a person may tend not to discuss their mental health challenges and ask for help (Gopalakrishnan, 2018). Furthermore, due to cultural influences on mental health, it may affect the way a person may describe their symptoms because of fear of being mistreated and discriminated.

Discuss why culture competence is important to nursing?

 Cultural competence is the mainstream of a great nurse- patient relationship. It helps the growth and development of nurses while also incorporating relevant societal values. Cultural competence is important in nursing care because it gives nurses the ability to provide the best health care to patients while demonstrating cultural awareness for their beliefs, race, and values. Cultural competence in nursing is necessary because it leads to significant satisfaction of care on the side of the patient and the key point is help nurses to have broader knowledge base of different cultures, communicate, and interact with their patient effectively.

What steps can a nurse take to be culturally competent?

Cultural competence is the capacity to recognize, respect and respond with appropriate words and actions to the needs and concerns of individuals from different ethnicities, social classes, genders, ages, or religions. The steps that nurses can take to be culturally competence includes cultural knowledge, cultural awareness, avoid making assumptions, build trust and rapport, educate patients about medical practices, and practice active listening (Sperry, 2012). Overcoming language barriers would also be beneficial for nurses to effectively communicate with patient by checking with facility for translator availability and using pictures and hand gesture to communicate when needed.

Great post. What designs were used within the articles that you reviewed? 

The identified problem can be categorized under qualitative design. The level of evidence that can be used to explain this problem is Level VI. First, it’s important to note that qualitative design research is specifically expressed in terms of words. It’s basically used for understanding concepts, thoughts or even experiences. This research helps in gathering an in-depth insights on the topics that are well understood. In the identified problem, the patients as well as families will be basically be the center of the research. The type of information that will be collected will actually depend on the decision that will be made by the patients as well as the family members. The healthcare quality will specifically depend on individual preferences depending on the standards of living and financial stability of a patient. Hence this is an indication that the information which will be collected will specifically qualitative. The methods to be applied in data collection for instance will be interviewing the patients with open ended questions, observing which will entail describing the situation in words, besides the exploration of the concepts and theories surrounding patients’ quality care (Peterson, 2019).

This is specifically the fundamentals of qualitative research design. The surveys are also other common method that will be applicable in data collection. This is because it contains a list of closed or multiple choice questions which are distributed to a sample of patients either directly or even via online. The Level IV evidence is chosen for it is actually an evidence from a single descriptive or qualitative study. This being a qualitative design, it qualifies this Level IV evidence to be one of the best evidence levels to address this identified problem (Bengtsson, 2016).
