Assignment: Applying Current Literature to Clinical Practice

Psychiatric mental health nursing practice is one of the newest disciplines to be licensed to provide psychotherapy As such, the majority of psychotherapy research is centered on other disciplines such as psychology, social work, marriage/family therapy, art therapy, psychiatry, and mental health counseling. This makes it essential for you to be able to translate current literature from other disciplines into your own clinical practice. For this Assignment, you practice this skill by examining the literature on group work and group therapy and considering its applicability to your own clients.

Learning Objectives

Students will:
  • Evaluate the application of current literature to clinical practice
To prepare:
  • Review this week’s Learning Resources and reflect on the insights they provide on group work and group therapy.
  • Select one of the articles from the Learning Resources to evaluate for this Assignment.

Note: In nursing practice, it is not uncommon to review current literature and share findings with your colleagues. Approach this Assignment as though you were presenting the information to your colleagues.

The Assignment

In a 5- to the 10-slide PowerPoint presentation, address the following:

  • Provide an overview of the article you selected, including answers to the following questions:
    • What type of group was discussed?
    • Who were the participants in the group? Why were they selected?
    • What was the setting of the group?
    • How often did the group meet?
    • What was the duration of group therapy?
    • What curative factors might be important for this group and why?
    • What “exclusion criteria” did the authors mention?
  • Explain the findings/outcomes of the study in the article. Include whether this will translate into practice with your own client groups. If so, how? If not, why?
  • Explain whether the limitations of the study might impact your ability to use the findings/outcomes presented in the article.

Note: The presentation should be 5–10 slides, not including the title and reference slides. Include presenter notes (no more than a half page per slide) and use tables and/or diagrams where appropriate. Be sure to support your work with specific citations from the article you selected. Support your presentation with evidence-based literature.

Conditions of the Ear

Objective data is the information gathered by the physician through the use of instruments and by assessing the normal and altered physiological functions in a patient. On the other hand, subjective data is what the patient says for instance how they feel and what could be the cause of the problem (Ozkaya et al., 2020). In this discussion, the focus is on the collection of both subjective and objective data in the diagnosis of a patient.

Another thing that the patient could explain is the time in which the pain started and how long it lasted before the drainage happened. By knowing when this pain started helps the physician to determine the extent to which the effect of the condition had gone as well as the kind of condition in which the patient is likely to suffer from. Another subjective data is the scale of the pain, on a scale of 1to 10 the patient can rate their pain so that the physician can know the kind of analgesics that the patient should be prescribed (Ozkaya et al., 2020). Another subjective data is the color and amount of the drainage from the ear so that if it was puss then they can be treated for infection, if it was blood then the physician can check for any injuries and in case of a clear fluid then there is need to assess the spinal cord integrity to determine what may be the cause of the cerebrospinal fluid flow.

On the objective data collection, the physician should use a source of light to visualize the internal ear so that a clear vision of what is wrong can be found. The lab examinations that can be done for this diagnosis are like culture and sensitivity for the fluid drained to be sure of the kind of infection or identify the DNA of the microorganisms. Another diagnostic test that can be done is the earing and balance test. These tests often are used to check on the efficacy of the auditory nerve which is responsible for hearing and transmission of sound waves (Abdullah & Hamed, 2020). The balance issue in the ear depends on the fluid found in the ossicles that are responsible for the balance in the cerebellum. In a case where the fluid draining from the ear is blood then there is a need to do some blood tests to determine some immune problems hence finding the necessary solutions to them.

The diagnoses that can be made based on the presentation the patient could be suffering from cerumen impaction which is defined as the accumulation of cerumen or rather ear wax in the ear preventing the assessment of the ear. Another diagnosis for this condition could be otosclerosis which is a condition in which there is abnormal bone growth around one of the small bones inside the ear(Abu-Naser,& Abu Hasanein, 2016). Another probable condition is menieres disease which a condition in which there is excess fluid in the inner ear. For our case, the patient claimed to have had drainage earlier before coming to the hospital.

For the case of cerumen impaction, the physician cannot visualize the internal ear because of the excessive ear wax. For this reason, it could be the wax that is causing the ear problem. For the case of otosclerosis, the physician claims that on trial to visualize the internal earl the bones were overgrown making it impossible to do so (Indius et al., 2018). And as for the menieres disease the patient claimed to have had a drainage that actually helped to ease the pain.

In conclusion, the ear may have several conditions depending on the presenting signs thus it is important to keenly obtain both the objective and subjective data to enhance appropriate ear problems (Indius et al., 2018).

WEEK 6: EVIDENCE SYNTHESIS

Critique the below evidence synthesis exemplar to address the following.

Patient falls with injury and fall prevention remain complex phenomena in the acute care setting as well as a major challenge for healthcare professionals (Gygax Spicer, 2017). Patient falls are considered one of the leading adverse events occurring in acute care settings such as hospitals and nursing homes, with the detrimental impact to the patient ranging from mild to severe bruising, fractures, trauma, and even death (de Medeiros Araújo et al., 2017). Falls are common phenomena in older adults, with roughly one out of three people age 65 years and older who suffers from at least one fall per year due to multiple factors including environmental, social, and physiological factors either alone or in conjunction (Gygax Spicer, 2017). The etiology is that patients are attempting to get out of bed without assistance from nursing staff. Several of the causative factors include illness, impulsiveness, urgency, medications, or being in an unfamiliar environment. Lastly, there has been an increase in the amount of turnover in staffing, thus reducing the amount of available nursing staff in the practice setting.

Does the author clearly identify the scope of the evidence synthesis? Explain your rationale.

  • Are strong paraphrased sentences included that are supported by contemporary sources of research evidence? Explain your rationale.
  • Are the facts related to the practice problem presented in an objective manner? Explain your rationale.
  • Does the author use sources to support ideas and claims, and not the other way around? Explain your rationale.
  • Based on your appraisal, is this exemplar a true synthesis of the evidence? Or is it a summary of the evidence? Explain your rationale.

Dq

Understanding the health care system at the local level is important to consider when planning an EBP implementation because it gives you an indication of what you can expect as far as demographics and availability of resources. According to my course preceptor, when any evidence-based implementation has taken place at her facility, such as the changed from paper charting to electronic medical records (EMR) has been a difficult endeavor that has required much planning and implementation. To promote use of research evidence in clinical and administrative health care decision making, multifaceted implementation strategies are needed (Moloney, Taylor & Ralph, 2016). Implementation strategies also need to address both the individual practitioner and organizational perspective. In addition to a shared vision, effective leadership is needed for change to be successful.

The key to transformational change or any change is to have leadership that is able to understand it, support it, explain it, and move the organization to commit to it (Cowell, Gillespie, Cheung & Brown, 2018). Although full schedules, distracting events, fear of change, and apathy are obstacles to change, the real enemy of change is complacency and having the will to change is vital even groups that seek change may be complacent and not committed to follow through.
