Case Study:

Richard is a 40-year-old man with a history of 4 weeks of cluster headache once each year. These began when he was 35 years old. His cluster periods occur in the fall. The cluster period begins slowly, increasing over 1 week, reaching a peak where Richard has two or three severe cluster attacks each day. They occur from 10 p.m. to 3 a.m. Each cluster headache lasts from 40 to 90 minutes, and the headaches are severe. The pain is always on the right side, with eye tearing and nasal congestion. 

Richard comes into our office 1 week into this fall’s cluster series. The headaches are increasing in intensity, and he is miserable with the pain.

NRS: 493 Professional capstone

In collaboration with the approved course preceptor, students will identify a specific evidence-based topic for the capstone project change proposal. Students should consider the clinical environment in which they are currently employed or have recently worked. Write a 500-750 word description of your proposed capstone project topic. Include the following:

  1. The problem or issue, intervention, quality initiative, educational need, or collaborative interprofessional team project that will be the focus of the change proposal.
  2. The setting or context in which the problem or issue, intervention, quality initiative, educational need, or collaborative interprofessional team project can be observed.
  3. A description (providing a high level of detail) regarding the problem or issue, intervention, quality initiative, educational need, or collaborative interprofessional team project.

Analyzing a Health Care Data Set

For this assessment, you will determine the relevant statistical tests to apply to the analysis of a data set, and then write a 3–4 page interpretation of the results of your analysis.

This assessment will ask you to select, apply, and interpret the results of a variety of statistical tests on a health care data set. This may include tests you have learned about or applied previously in the course, or the new nonparametic t-Test which is presented in the resources for this assessment. The challenge is using what you have learned to determine the best course of action to complete the interpretative tasks the assessment lays out for you. This attempts to mirror real-world situations where the data or statistical analysis could be approached in a variety of different ways. To decide which statistical test to use for the various dependent variables to be analyzed, one must first know more about the data type (measurement level) within those variables.

Overview

Public health researchers are often involved in collaborating in the design, development, and analysis of community initiatives of varying complexity. While this course alone will not provide sufficient training for you to act as a statistical consultant, it does offer a broad and practice-based analytic foundation that can position you to better understand and more fully contribute to real-world project teams. Building on the basic statistical concepts and analytical techniques of the previous units, this assessment is an opportunity to use your cumulative quantitative-analysis skills to address a broad set of real-world research questions.

Part 1: Yoga and Stress Study Statistical Tests
  1. Use the Yoga Stress (PSS) Study Data Set [XLSX] to determine the measurement level of data of the dependent or outcome variable (Psychological Stress Score) you are analyzing. 
    • Is the data categorical, ordinal, or interval or ratio?
  2. Before performing any statistical tests, you must determine which tests would be most appropriate for your data type. 
    • Perform a preevaluation of the data for outliers (all variables) and normal distribution (only dependent variables) as you have done previously.

    • Use the readings, media, resources, and textbook as guides to perform an analysis of the selected variables.
  3. Perform and interpret an appropriate series of statistical tests (including preanalytical testing for outliers and normal distribution of data) that answer the following research questions: 
    • How would you quantitatively describe the study population? 
      • Summarize the primary demographic data using descriptive statistics.
    • Is there any association between gender and race in this military study? 
      • Perform an appropriate chi-square analysis.
  4. Perform preliminary assessment of the data, then compare pretest to post-test scores. 
    • In total population being studied, what was the effect of the yoga intervention on stress?

Employee Retention Issues in Workplaces

Employee retention in workplaces remains one of the main issues human resource managers are dealing with for decades. That is why, in 2016, the Society for Human Resource Management (SHRM) reports that 46% of HR managers deem employee turnover as their top concern (Lee et al., 2018). Therefore, many programs have been experimented to improve employee’s turnover in workplaces. This situation is also common in the healthcare system. This situation has brought many harmful consequences to workplaces. The lack of experienced workers and the increase in the cost of human resources in new employees’ orientation are the main issues that workplaces are dealing with regarding employee turnover. That is why it is understandable that the issue presented in chapter 60 of the textbook refers to programs that may help retain health care professionals in workplaces.

The main issues that make up the author’s concern are reducing employee turnover in healthcare workplaces and retaining nurses in those settings. Their approaches were trying to find some loyalty programs that may help experienced nurses and new nurses.

The finding was not easy since a program that may work for new nurses may not impact professional nurses’ interest. For instance, a study published in the American Journal of Preventive Medicine has shown that young nurses from 33 years old or younger do not plan to stay in workplaces and are determined to have their own business (Sellers et al., 2020). That is proof that policies that address the interest of experienced nurses may not influence younger nurses. One focus of professional nurses is a healthy workplace. If the environment does not protect employees, experienced nurses may leave their workplaces and look for a better one. This health protection may include better health insurance provided by the workplace.

 The student believes that those issues persist in workplaces today. Many reasons make her understand that workplaces are still suffering from this persistent situation. If we analyze the causes, one should admit that young nurses are dealing with issues like the need to feel part of an organization that will help them grow professionally. They also have concerns about their childcare system. However, they share with the experimented nurses the need for the management to value them. Those are current issues that the healthcare workplaces are dealing with. As good responses to those issues, the enforcing of the emerging nurse leader program and the magnet recognition program may bring solutions that meet nurses’ interest in the current healthcare workplaces

We can make a difference in participating in nurses’ organizations that provide ideas to help sustain the healthcare system with experienced and educated nurses. That will help solve the issues related to nurse’s turnover in workplaces that will positively impact the patient outcome.
