Instructions from my professor:

Soap Note 1.  Heart Failure.

Must use the sample template for your soap note. 

Follow the MRU Soap Note Rubric as a guide

Use APA format and must include minimum of 2 Scholarly Citations.

TURN-It-In (anti-Plagiarism program) will be used.

The use of tempates is ok with regards of Turn it in, but the Patient History, CC, HPI, The Assessment and Plan should be of your own work and individualized to your made up patient. 

You must use template for Soap note 1. Use the information provided in the PDF as an “EXAMPLE”. You must use the word temple file and tailor it to your patient and their diagnosis. Using this template will make it very easy to learn the things needed in the note and help you not forget to put something or lose points due to incorrect format or missing information. 

The Chief Complaint, Patient info, HPI, Plan section, and references must all be of your own work and no copy-paste.

The Main Areas of Focus that will be checked for plagiarism is Chief Complain, History of Present Illness (HPI), Assessment with Rationale and Explanation, and the Plan. All of this should be in your own words and not copy-pasted from a past note or website or book. There should be minimum likeness noted by turn it in software in these areas. 

The Objective and Subjective information can be from a template (Standard Documentation) and will only be looked at for content and not for plagiarism. So if you are past 50% and the above main areas of focus sections are clear and minimum then you are ok. You can resubmit as many times but after 3 it takes up to 24 hours to get turn it in a score.

This is a made-up patient, so review your diagnosis and have the patient have the standard presentation, objective and subjective symptoms that would typically present and adjust them on.

10. 200 words—Locate a research article pertaining to schizophrenia disorder as it pertains to the medias perception of this disease, what role do community and public health nurses play in minimizing the negative stigma surrounding this mental illness, provide examples?

 For this assignment you will provide a detailed discussion of a theory of pop culture and provide critical discussion of this theory. The purpose of this assignment is to help you hone your critical thinking skills. You will provide exposition and criticism of a theory of pop culture. You may choose either Mass Cultural Theory or Critical Theory.

Your essay should include some discussion of the following questions:

1. What or who determines popular culture? Where does popular culture come from? Does it emerge from the people themselves as an autonomous expression of their interests and modes of experience, or is it imposed from above by those in positions of power as a type of social control?

2. Describe how commercialization and industrialization influences popular culture. Does reducing culture to commodity mean that the criteria of profitability and marketability take precedence over quality, artistry, integrity and intellectual challenge? Or does it result in a universal market where what is popular actually corresponds to what people want and think is valuable? Does marketability and profitability actually produce quality art or does it only benefit business interests?

3. Is popular culture used to control and indoctrinate the people? Is it used to get people to accept and adhere to ideas and values which will ensure the continued dominance of a particular elite and allow them to exercise power over them? Or is popular culture about rebellion and opposition to the prevailing social order? Is pop cultural a spontaneous genuine burst of creativity or is it just a means through which a dominate group exercises control of society?

4. After you provide an accurate account of one of these theories of pop culture, you should then review the strengths and weaknesses of the theory. Can you identify some objections to the theory? You can research objections, but you need to consult reputable sources.

The Office (2005-2013), was a mockumentary television series that featured a myriad of hilarious characters.  Watch the videos below that showcase the show’s characters; Kelly Kapoor, Michael Scott, and Dwight Schrute.  For each character, select a personality disorder that could be diagnosed based on their behavior in the video.  Provide specific examples that made you make this decision.  There’s no right or wrong answer.  This is just an exercise to familiarize yourself with the behaviors of individuals with personality disorders. The DSM-5 should always be used as a tool to help others, not to weaponize or stigmatize others.  If you are unfamiliar with the show, pictures below state who Kapoor, Scott, and Schrute are in the videos.
