Work with your preceptor to assess the organization for required resources needed for the strategic plan if the change proposal were to be implemented. Review your strategic plan and determine what resources would be needed if the change proposal were to be implemented. Write a list of at least four resources you will need in order to implement your change proposal.

Standards of practice

Chapter 3 describes Standards of Practice. Create a PowerPoint or Prezi describing the 6 Standards of Practice and the 11 standards of professional performance. There should be one slide for each standard, except for #5, which should have 4 slides. See Table 3.1. Assignment Requirements 

  1. Please go to a lay press newspaper, for example, The Washington Post, The New York Times, or other national publication. 
  2.  Locate an article that has global health implications, such as malaria, untreated strep throat or other infectious diseases in developing countries.  Other topic areas may address environmental global health issues, for example, safe water, sanitation, or oral health.   
  3. After you locate and read over the article, respond to the following prompt:
    • During NR503, we have discussed the web of causation, epidemiological triangle, inter-professional collaboration and various other concepts related to population health and epidemiology.  In responding to your chosen article in this week’s discussion board be sure to synthesize, integrate, these course concepts as well as other terminology found in weekly modules. For example, integrate into your writing information from the World Health Organization and the SDG’s or include social justice theory into your article analysis.  The specific focus is up to you, but be sure to compare and contrast, integrate and think critically, as you present your discussion board post. 
  4. Your post should be a minimum of three (3) paragraphs with 4-5 sentences per paragraph.

Read the following case study and submit a 1500 – 2000 word paper that addresses the case study and the questions at the end of the case study. 

Mr. B. is an 83 year old male referred to home health services following a hospital admission for congestive heart failure. His payor source is Medicare (traditional). He lives alone and indicates he does not have sufficient financial resources to afford private-pay in home supportive care. His daughter, who lives in the neighboring county about 30 miles away, has offered her father to move in with her – he has refused and she is not “forcing the issue”. Mr. B. is also not open to discussing moving to an assisted living environment.

This is his third hospital admission for the same problem over the last 12 months and his second admission to a home health agency. He was discharged from the previous home health agency 6 weeks ago. Physician orders are for the home health agency to provide skilled nursing for assessment of his cardiac condition and to teach him about his care and medication regime. Physical therapy has been ordered to establish a home exercise program. Home health aide services have been ordered 3 times a week to assist Mr. B. with his personal care needs.

By the end of the third week of service, it is clear to the RN Case Manager that Mr. B. is not making progress to the established goals. She reports during case conference that he is not a good candidate for teaching because of intermittent confusion and forgetfulness. According to the nurse, no matter what she has tried to teach him and regardless of the aids she has used, Mr. B. continues to take is medication incorrectly. She states, “I know he needs services, but I am not sure that he is in the right environment. I am concerned about Mr. B. – I am not sure why his daughter won’t just step up and put her foot down.”

The physical therapist reports at case conference that the “client lacks motivation”. The home exercise program has been established with Mr. B.; however, he does not complete the exercises between therapy visits.

An improvement in his personal hygiene has been noted with the Home Health Aide (HHA) services. The HHA has also been successful in decluttering his living space. The HHA reports that she is concerned that he is not eating regular meals.

This situation offers an ethical challenge to the professional caregivers and to the agency for which they work. 

Discuss how you would handle this ethical dilemma. Address the following questions as a part of your response:

Let’s return to the ethical scenario presented within Page 2 of the Explore section of Week 4 to consider it further. Upon learning that a medication error occurred resulting in a child’s death, the Chief Nursing Officer (CNO) of a regional acute care facility held a press conference to announce the mistake. The conference centered on reporter questions about why the nurse was not fired. The CNO explained that the nurse was highly regarded and that the problem was a “system” problem, not simply a problem of an incompetent nurse. The CNO also explained that an examination of the medication administration procedures was ordered and that an electronic medication error avoidance system would be implemented to ensure that such an error would not happen again. Consider this scenario to address the following.

· As the CNO in the scenario, defend your decision to disclose the incident to the media. How does this decision influence value-based healthcare? What risks might you encounter? What impact might this decision have on you personally and professionally?

·  At a regional health consortium, fellow CNOs remark that they would not disclose such an incident to the media. Provide two justifications for withholding disclosure of the error to the public. How does this decision influence value-based healthcare?

·  Imagine you are the patient’s family. How would you want the issue to be handled by the CNO? Explain your rationale
